GENERAL PRICE LIST

These prices are effective as of February 11, 2011

 but may be subject to change without notice.

In compliance with the trade regulations of the Federal Trade Commission(16CFR Part 453), we are required to provide you with this General Price List.  The goods and services shown below are those we provide to our customers.  You may choose only the items you desire.  However, any funeral arrangements you select will include a charge for basic services and overhead.  If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

PLEASE NOTE:  THIS LIST DOES NOT INCLUDE PRICES FOR CERTAIN ITEMS YOU MAY ASK US TO BUY FOR YOU, SUCH AS CEMETERY OR CREMATORY SERVICES, FLOWERS AND NEWSPAPER NOTICES.  THE PRICES FOR THOSE ITEMS WILL BE SHOWN ON YOUR BILL OR THE STATEMENT DESCRIBING THE FUNERAL GOODS AND SERVICES YOU SELECTED.

  Basic services of Funeral Director and Staff……………$1,900.00 *

      Our basic service charge includes, but not limited to: staff to respond to initial request for services; 

      arrangement conference with family or responsible party; preparation and filing of necessary

     certificates and permits; shelter of remains; planning of funeral arrangements; and coordination of

      service with cemetery, crematory, vault company and others as required.  Also included in this charge

      are overhead expenses relative to our facility such as insurance, maintenance, utility expenses,

      administrative costs and equipment expenses.  This fee for our basic services and overhead will be

      added to the total cost of the funeral arrangements you select.  (This fee is already included in our

      charges for direct cremations, immediate burials and forwarding or receiving remains.)

  Embalming…………………………………………$590.00 *

      Except in certain special cases, embalming is not required by law.  Embalming may be necessary;

      however, if you select certain funeral arrangements, such as a funeral with viewing.  If you do not

      want embalming, you usually have the right to choose an arrangement that does not require you to

      pay for it, such as direct cremation or immediate burial.

   Autopsy Procedure Restoration…………………$100.00

  Other Preparation of the Remains…………………$295.00 *

     This charge includes, but is not limited to: necessary cosmetic work, dressing and casketing of the

     deceased.  This charge does not include the charge of the hairdresser.

  Use of Facilities, Staff and Equipment for Services Conducted at the Funeral Home or Another Facility

 Use of facilities and staff for visitation…………………$595.00 *

  Use of facilities and staff for funeral ceremony………...$595.00 *

Use of facilities and staff for additional day of visitation (or portion thereof)………$395.00

     Memorial service when disposition takes place elsewhere;

this charge includes basic service of the  funeral director and staff, preparation of notices and

coordination of service plans with those parties involved.

Forwarding Remains to Another Funeral Home……………………$1,795.00

    This charge includes removal of remains, services of staff, embalming, necessary authorizations,

    local transportation to the airport or railroad.  This service does not include the cost of the air tray,

    combo unit, shipping charges, use of facilities for viewing or ceremonies.

Receiving of Remains from Another Funeral Home……………....$1,625.00

   This service includes a basic service of staff, local transportation of remains by service car to the

    cemetery or crematory and/or required services of staff.  This does not include other services of

    staff or the use of facilities for viewing, funeral rites, or ceremonies.  This does not include

    merchandise, taxes, or cash advances.

Cremations Services:  Range in price from………$1,250.00 to $6,650.00

    Our charge for Cremation Service includes: local transfer of remains from the place of death to

    our funeral home, funeral counseling and arrangements, recording of the vital statistics, securing

    and filing of the death certificate and cremation permits, transportation to the crematory and the

cremation expense.

    This charge does not include any use of facility, container, medical examiner's fee or cash advance

item charges. If you want to arrange a direct cremation, you can use an alternative container.

Alternative containers encase the body and can be made of materials like fiberboard or

composition materials with or without an outside covering.

    Cremation with the use of a casket……………………$1,745.00

      Casket Description:  K-Flare with twill interior

    Cremation with use of an Alternative Container……$1,445.00

       Container Description:  Plywood base, fiberboard top and sides, unfinished interior

    Cremation with Casket or Container provided by client…$1,250.00

Immediate Burial:  Range in price from………$2,045.00 to $7,445.00

   Our charge for an Immediate Burial includes:  local transfer of remains from the place of death

    to our funeral home, funeral counseling and arrangements, recording of vital statistics, filing of

    the death certificate and use of the funeral coach to a local cemetery.  This charge does not

    include any use of facility or cemetery charges.

    Immediate Burial with use of Casket…………………$2,540.00

      Casket Description:  K-Flare with twill interior

    Immediate Burial with Casket provided by the purchaser…$2,045.00

Anatomical Donation…………………………………………$1,295.00

    Our charge includes local transfer from the place of death to our funeral home, arrangement

    counseling, obtaining and filing of legal documents and transportation to a local Medical School.
